(Funded rate structure) Individuals who have active coverage without Unlimited Duration prior to 01-01-2020
Class 002: Closed Class as of 01/01/2020
(Funded rate structure) Class for those who elected coverage on or after 01-01-2020 and prior to 01-01-2021 (without Unlimited)
Class 003: (Active) All full time and part time employees working at least 20 hours per week and their family members and domestic partners.
Individuals who are not covered and are applying as a new enrollee on or after 01-01-2021
Class 004: Closed Class as of 01/01/2021
(Funded rate structure) Class for those who had elected unlimited duration prior to 01-01-2021
During the enrollment process, you will receive and acknowledge receipt of additional notices and disclosures
You will receive a full copy of your policy from the insurance company if your enrollment is approved. It is your responsibility to confirm that you have received policy documents and that payroll deductions have occurred. If you have not received policy documents and you do not see a payroll deduction within 30 days, then you may not have active coverage. Call LTC Solutions, Inc. immediately at (877) 286-2852.
